Fannin County SSA, works with each student to find success, not only during school years, but also as they transition beyond school to adulthood. The transition planning begins during a student’s annual Admission, Review, and Dismissal (ARD) meeting when the student turns 14 years old. FCSSA helps students set goals toward their future after graduation. These goals are included in the student’s Individualized Education Plan (IEP) as part of the transition plan.
FCSSA also helps with the following as part of the transition services: planning high school courses based on a student’s goals; school counselors, teachers, and special education personnel help the student and family plan to meet the goals set by the student; invite outside agency representatives to ARD meetings to assist in planning for transition services; and each year at the annual ARD meeting, a review of the student’s post-graduation goals are discussed and high school classes and transition services are modified according to the needs of the student.
